This registration session will allow you to complete contact information, sign up for participation, and submit payment.

Please read through each page carefully to ensure that all information is provided accurately. 

You will need the following items to complete registration:

Participant Photo

Birth Certificate

Most Recent Report Card

TWO Proofs of Residence (Documents must include physical address with current dates)

Emergency Contact Information

* Please Note: When uploading documents, please do NOT use HEIC files (jpeg, png, pdf, etc only) *

To complete registration through our secure site, please have your Visa, MasterCard, Discover, or ACH Bank Account information available. A mail-in option for check payment is also available. All Discounts will be applied at checkout. If you have a football and a cheer player and your sibling discount isn't showing up please email Mary @ treasurer@5cyfl.com
